One of the most frustrating situations for people with hypothyroidism is seeing their blood test results come back with an elevated TSH despite taking levothyroxine exactly as prescribed. The natural reaction is often: "How can this be happening? I haven't missed a single tablet."
It's a completely reasonable question, and if you're not sure what the number itself actually reflects, this guide on what an elevated TSH means is a good place to start. Most people assume that if they take their medication every morning, their thyroid blood tests should automatically return to normal. In reality, thyroid physiology is far more complex than that.
Levothyroxine replaces the hormone your thyroid is unable to produce in sufficient amounts. What it cannot do is guarantee that your body absorbs it properly, converts it efficiently, or uses it as effectively as it should. This is why two people on exactly the same dose can have completely different blood test results and very different symptoms.
Taking Levothyroxine Correctly Matters More Than Many People Realise
The first thing I usually discuss with clients is not the dose but how they take their medication.
Levothyroxine is absorbed mainly in the small intestine, and that process is surprisingly sensitive. Even small daily habits can reduce absorption without people realising it.
Coffee is one of the most common examples. Drinking coffee too soon after taking levothyroxine can reduce the amount of medication absorbed. Calcium supplements, iron supplements, magnesium, high-fibre supplements and some medications for acid reflux can have a similar effect if taken too close together.
Ideally, levothyroxine should be taken with water on an empty stomach, with enough time before breakfast or coffee. If supplements containing iron or calcium are needed, they should usually be taken several hours later.
For some people, simply improving medication timing is enough to see TSH begin to normalise.
Your Dose May No Longer Match Your Body's Needs
Thyroid hormone requirements are not always constant throughout life.
Body weight can change. Pregnancy increases thyroid hormone needs. Menopause, ageing, gastrointestinal conditions and new medications may also influence how much levothyroxine is required.
Sometimes an elevated TSH simply reflects the fact that the current dose is no longer appropriate. This is one of the reasons why regular monitoring with your GP remains important, even when you've been stable on the same dose for years.
Your Digestive System May Not Be Absorbing Levothyroxine Properly
This is a factor that deserves much more attention than it often receives.
Levothyroxine has to be absorbed through the digestive tract before it can work. If digestion is compromised, absorption may become less efficient.
Conditions affecting the stomach or intestines, low stomach acid, chronic inflammation, changes in the gut microbiome, or persistent digestive symptoms may all influence how much medication actually reaches the bloodstream.
In my clinical practice, digestive health is always part of the conversation because it may affect how well levothyroxine is absorbed. If the medication is not being absorbed efficiently, it cannot have its full effect.
TSH Is Only One Part of the Picture
Many people focus entirely on TSH because it's often the number highlighted on laboratory reports. While it's an important marker, it does not tell the whole story.
Whenever possible, it's worth asking your GP whether Free T4 and Free T3 can be checked alongside TSH. These hormones provide additional information about how thyroid hormone is circulating and being converted in the body.
It is not unusual to see someone with a TSH that is still above the desired range while Free T4 has improved. Equally, some people have a normal TSH but continue to experience symptoms because other factors are contributing to how they feel.
Blood tests should always be interpreted alongside symptoms rather than in isolation.
Stress Can Influence Thyroid Function
Stress is rarely the sole explanation for an elevated TSH, but it can make thyroid function more complicated.
Long-term stress affects the communication between the brain and the thyroid. It can influence thyroid hormone conversion, alter cortisol patterns, and affect sleep, digestion, appetite, and overall metabolism.
I often meet clients who are taking their medication perfectly but are sleeping five hours a night, working under constant pressure, and feeling mentally exhausted. In these circumstances, it's hardly surprising that they continue to feel unwell, even when medication has been prescribed appropriately.
Supporting stress management is an important part of improving overall thyroid health. It may not change your prescription overnight, but it can have a meaningful impact on how you feel and how well your body functions.
Nutrient Deficiencies May Also Play a Role
The thyroid depends on a number of nutrients to function well. Iron, selenium, zinc, iodine, vitamin D, and adequate protein intake all contribute to healthy thyroid physiology.
One of the most common patterns I see in practice is insufficient protein intake. Protein provides the amino acids needed for many metabolic processes, including those involved in hormone production and transport. Low iron stores are another frequent finding, particularly in women with heavy menstrual periods.
Correcting these deficiencies does not replace levothyroxine, but it may help the body use thyroid hormones more efficiently and improve overall wellbeing.
Sometimes the Answer Is Simply That Your Treatment Needs Reviewing
There is no perfect levothyroxine dose that lasts forever.
If your TSH remains elevated despite taking your medication consistently, it does not automatically mean you're doing something wrong. It means there's a reason that deserves investigation.
Sometimes the dose needs adjusting. Sometimes medication timing needs improving. Sometimes digestive health, nutrient status, or lifestyle factors are contributing more than expected.
Rather than assuming you need a higher dose immediately, it's worth stepping back and looking at the whole picture.
