High Thyroid Antibodies (TPO & Tg): What Do They Really Mean?
What it means when TPO or Tg antibodies are high but your TSH is still normal — and what you can actually do about it.

One of the most common things I hear from new clients is:
"My thyroid antibodies are very high, but my doctor told me my thyroid hormones are still normal. He said we'll just repeat the blood tests in six months. Is there really nothing I can do?"
I completely understand why this feels frustrating.
On one hand, you have blood tests showing that your immune system is already reacting against your thyroid. On the other hand, you're being told to wait because your TSH, Free T4 and Free T3 are still within the laboratory reference range.
From a conventional medical perspective, this approach makes sense. Thyroid medication is prescribed when the thyroid can no longer produce enough hormones. If your TSH, Free T4 and Free T3 are still within range, there is often no medication to prescribe.
As a nutritional therapist, however, I see this stage very differently.
For me, elevated thyroid antibodies are not simply another number on a laboratory report. They tell me that the autoimmune process has already started, even if your thyroid is still managing to keep hormone production within the normal range.
And that matters, because this is often the stage where we have the greatest opportunity to support the body.
Elevated TPO or Tg antibodies mean your immune system has already started reacting against your thyroid, even if TSH, Free T4 and Free T3 are still completely normal. That stage often gets left unaddressed, but it's frequently the best window to support the immune system before more thyroid function is lost.
What are thyroid antibodies?
The two antibodies most commonly measured are thyroid peroxidase antibodies (TPO antibodies) and thyroglobulin antibodies (Tg antibodies).
These antibodies are produced by the immune system. Instead of recognising only bacteria or viruses, the immune system has started reacting against proteins within the thyroid gland.
This does not happen overnight. Autoimmune thyroid disease usually develops gradually, often over many years, before thyroid hormone levels begin to change.
That is why someone can have positive TPO antibodies while TSH, Free T4 and Free T3 are still completely normal.
In other words, the autoimmune process may already be present long before hypothyroidism develops.
Why high antibodies matter even if your TSH is normal
This is probably the most important message I want people to understand.
A normal TSH does not mean that nothing is happening.
It simply means that, at this moment, your thyroid is still producing enough hormones to keep up with the body's demands.
The thyroid has an impressive ability to compensate. Even while the immune system is creating inflammation within the gland, the remaining healthy thyroid tissue can continue producing enough hormones for months or even years.
Eventually, if enough thyroid tissue is affected, the gland can no longer compensate. This is when TSH begins to rise and hypothyroidism becomes apparent.
From my perspective, waiting until this stage before paying attention to overall health means missing an important opportunity.
Why many doctors do not repeat thyroid antibodies
People often ask me why their doctor measured antibodies only once.
The answer is actually quite straightforward.
Conventional medicine currently has no medication designed specifically to lower thyroid antibodies. Levothyroxine replaces thyroid hormone when it becomes necessary, but it does not stop the autoimmune process itself.
For that reason, treatment decisions are usually based on TSH and thyroid hormone levels rather than antibody levels.
This does not mean antibodies are unimportant.

This is where nutritional therapy becomes important
When someone comes to me with elevated TPO or Tg antibodies, my first question is never, "How do we lower the antibodies?"
Instead, I ask a different question.
Why is the immune system under so much pressure?
The answer is different for every person.
Some people have long-standing digestive symptoms that have never been properly investigated.
Others are sleeping five or six hours a night while living under constant stress.
Some have significant nutrient deficiencies that have developed over many years.
Others have diets that appear healthy but contain very little protein, making it difficult for the body to repair tissues and maintain normal immune function.
This is why I never look at thyroid antibodies in isolation. They are part of a much bigger picture.
Supporting the immune system means supporting the whole body
Hashimoto's is an autoimmune disease.
The thyroid is the organ being affected, but the immune system is driving the process.
That is why my consultations rarely focus only on the thyroid.
We spend time discussing digestion because around 70% of immune cells are associated with the gut through the gut-associated lymphoid tissue.
We look at liver health because the liver plays an important role in thyroid hormone metabolism and overall inflammatory balance.
We review protein intake because it is one of the most common nutritional gaps I see in practice.
We assess nutrients such as selenium, iron, zinc and vitamin D, because deficiencies are common and each of them contributes to normal immune and thyroid function.
And we talk about stress.
Not because stress "causes" Hashimoto's, but because living in a constant state of physical or emotional stress makes it much harder for the immune system to settle into a healthier balance.
None of these factors acts alone. Together, they influence the environment in which the immune system is functioning every single day.
Can thyroid antibodies be reduced?
This is probably the question I am asked most often.
The honest answer is yes, they often can.
In clinical practice, it is not unusual to see TPO or Tg antibodies gradually decrease as overall health improves. This does not happen because of one supplement, one superfood or one restrictive diet. It usually happens because several areas are addressed at the same time.
Better nutrition. Improved digestive health. Correcting nutrient deficiencies. Supporting healthy sleep. Reducing chronic stress. Creating a lifestyle that gives the immune system a chance to function more calmly.
Will this happen for everyone? No.
Can autoimmune thyroid disease be cured? Unfortunately, no.
But many people experience lower antibody levels, more stable thyroid function and, most importantly, a significant improvement in how they feel. Autoimmune diseases may not disappear, but they can become much quieter, and periods of remission are absolutely possible.
The bottom line
High TPO or Tg antibodies are not just another laboratory number to ignore.
They tell us that the immune system has already begun reacting against the thyroid, even if thyroid hormone levels are still within the normal range.
From a medical perspective, this stage is often monitored until treatment becomes necessary.
From a nutritional therapy perspective, this is also the moment to start asking a much bigger question: what can we do to support the immune system before more thyroid function is lost?
That means looking beyond the blood test. It means understanding your diet, your digestive health, your sleep, your stress levels, your nutrient status and the many everyday factors that influence how your immune system functions.
The goal is not simply to lower antibodies. The goal is to create the best possible environment for your immune system to become calmer, to preserve thyroid function for as long as possible, and to help you feel healthier, stronger and more like yourself again.

Can thyroid antibodies be high with a normal TSH?
Yes. The thyroid can compensate for ongoing immune activity for months or years before hormone levels change, so TPO or Tg antibodies can be elevated long before TSH rises.
Why doesn't my doctor retest my thyroid antibodies more often?
